During IFR practice ride today controller advised us to "report established in the holding pattern". Does this mean upon crossing the fix for the first time (to begin, for example, a parallel entry) as in "report entering holding" or when you are on the holding course inbound as in "maintain x feet until established on the localizer inbound..." or after you cross the fix (assuming, for example, a parallel entry) and begin the outbound turn?? Any thoughts?

Yes, this is the answer I chose. (BTW, I am a CFII). My student asked the question and, like so many of us old timers, I simply recited the answer and only when he asked "why" did I stop to think about it. The "protected airspace" concept is the key as you point out. In a non-radar environment the phraseology would have been different certainly.
